Job Details
What You Will Be Doing
Conduct initial and reassessment visits for home care patients
Perform UAS evaluations (preferred but not required)
Initiate and update care plans under physician orders
Supervise and coordinate Home Health Aides (HHAs)
Monitor patient progress and communicate changes to physicians
Educate patients and caregivers on treatment plans and medications
Travel to multiple patient homes daily across NYC boroughs
Skills You Need
Active New York State RN License
1+ year experience in hospital or home care settings
COVID-19 vaccination required
Strong communication and time-management skills
Ability to work independently and travel locally
UAS certification preferred
Bilingual fluency is a plus
