We are seeking and have an immediate need for a Physician Assistant / Nurse Practitioner to become a part of our team! You will focus on providing high quality patient care as part of a healthcare team to help treat patients with a history of substance abuse with use of MAT( Medication Assisted Treatment) along with patients seeking screening and treatment for Hepatitis C.

Responsibilities :

• Diagnose, treat and provide medication management / injections for patients with Substance Use Disorder alone or in combination with concurrent psychiatric illness and hepatitis C
• Prescribe and perform all necessary treatments
• Perform comprehensive physical examinations of patients
• Implement practice established patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities when necessary
• Communicate with collaborating physician or specialist regarding patient care
