Occupational Therapist Assistant (COTA) Per Diem

Alliance Health at Baldwinville is a 94-bed long term / rehab care facility located in Baldwinville, MA. We are looking for a per diem COTA to join our Rehab team.

Responsibilities Include :

Performs chart review, obtaining information pertinent to resident’s plan and program.   Reports findings to OTR.

Performs Activities of Daily Living (ADL) evaluation - addressing areas of grooming, hygiene, bathing, dressing, homemaking, and functional mobility.   Confers with OTR, determines problem list, short term goals, long term goals, and plan of treatment

Baldwinville, MA : Reliably commute or planning to relocate before starting work (Required)

License / Certification : Occupational Therapy Assistant License (Required)
