Job description

We are seeking an experienced and motivated Registered Nurse (RN) Case Manager to oversee our care delivery team and ensure high-quality execution of our PCM / CCM programs.

In this role, you will supervise Licensed Practical Nurses (LPNs), review and sign off on care plans, maintain clinical quality standards, and ensure compliance with Medicare guidelines. You'll play a critical role in bridging patient care, clinical operations, and compliance, ensuring our patients receive consistent, proactive, and coordinated support between visits.

This is an on-site role based in the San Francisco Bay Area, with flexibility for hybrid or remote work.

Key Responsibilities

  • ? Oversee and support Licensed Practical Nurses in the delivery of care coordination, patient engagement, and documentation tasks.
  • ? Create, review, approve, and sign off on care plans and documentation.
  • ? Conduct quality assurance reviews to ensure accuracy, timeliness, and completeness of clinical records, while ensuring compliance with Medicare PCM / CCM requirements.
  • ? Serve as the clinical liaison between our team and partner neurologists, ensuring effective communication and escalation of clinical concerns.
  • ? Provide training, mentorship, and feedback to Licensed Practical Nurses to promote ongoing professional development and care excellence.
  • ? Participate in workflow optimization and quality improvement initiatives with the care operations and AI teams.
  • ? Engage patients and caregivers periodically to assess care quality and address escalated clinical needs.
  • ? Maintain detailed time-based activity logs for compliant CPT billing and documentation.
  • ? Uphold our company's standards of compassion, professionalism, and proactive care delivery.

MUST HAVES

  • CA State License
  • 2+ years in chronic disease management or neurology (Alzheimer's, dementia)
  • Exp supervising or mentoring clinical staff
  • PLUS

  • Medicare documentation and billing
  • Alzheimer's, dementia exp
  • Telehealth exp
  • Multistate licensure
  • $115,000 to $130,000 per year annual salary.
