Position Overview

We are is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. This role provides professional geriatric nursing care, treatment, and services for our Veteran residents. The LPN will oversee and train subordinate staff, deliver hands-on nursing care, and ensure all residents receive high-quality, individualized care in accordance with established standards and regulations.

Pay Rate : $44.45 per hour.

Schedule :

12-hour shifts (6 : 00 PM – 6 : 30 AM)
Must be available to work weekends
Block booking schedule :

Week 1 : 36 hours

Week 2 : 48 hours

Key Responsibilities

Administer medications and perform physician-ordered treatments within the LPN scope of practice (oral, enteral, parenteral, suppository, wound care, tracheostomy, gastrostomy, catheter care, etc.)

Observe, document, and report resident conditions and treatment effectiveness

Maintain adequate medication and supply inventory

Participate in shift reports and communicate effectively with residents, families, and staff

Complete initial admission assessments and assist with ongoing care plans and evaluations

Supervise subordinate staff, including CNAs and LPNs, when assigned as charge nurse

Assist residents with Activities of Daily Living (ADLs) as needed

Participate in staff meetings, in-service training, and professional development

Collect and analyze data for quality improvement and infection control programs

Respond promptly and appropriately to emergencies

Train and mentor new employees in facility procedures and standards

Knowledge & Skills

Geriatric and gerontological nursing theory and practice

Understanding of the aging process and changing resident needs

Team nursing principles and interdisciplinary care coordination

Pharmacology, infection control, and medication administration procedures

Federal and state nursing regulations and best practices

Strong leadership, communication, and problem-solving abilities

Ability to assess, plan, and evaluate resident care effectively

Requirements :

Active LPN License (State of Arizona)

Current CPR / BLS Certification

Level 1 Fingerprint Clearance Card

7-Year Background Check and Drug screen

Current TB Skin Test within six months of hire
