WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) …

We are seeking an experienced case manager to join our team. The Case Manager is responsible for actively engaging and collaborating with clients to reduce barriers to achieving positive outcomes in critical areas of life.

The program will help residents maintain stable housing and improve overall wellness.
