LPN- Wound Care Nurse

The LPN Wound Care Nurse is responsible to assist wound nurse with rounds, will be responsible for completing all major treatments in the facility, including designated by wound nurse and DON- assist to ensure care plans are up to date and completed with appropriate interventions. Responsible to ensure all treatment carts are stocked and in place to provide treatment as prescribed, assist with MD orders.
RESPONSIBILTIES :
Wound Treatments : Under the guidance of the RN Wound care nurse, the primary responsibility of this position is the completion of wound treatments in accordance with standards of practice and physicians’ orders for ALL neighbors in the facility with wounds
Infection Control : Support infection control efforts as it relates to both wound care and the facility’s overall infection control program
Wound Rounds : Performing weekly wound rounds with the interdisciplinary wound care team. Ensure new wound treatment orders are carried out, wound treatments completed, care plans updated to reflect the current status of each wound and that the responsible party is kept abreast of the status of improvement / declination of all wounds. Serve to assist in the reduction of nosocomial wounds through implementation of preventive measures. Perform weekly skin checks as assigned.
Negative Pressure Therapy : Perform and / or troubleshoot negative pressure therapy including assisting and educating floor nurses on accurate completion and documentation of the same in accordance with professional standards of practice
QAPI Participate in the facility’s quality assurance and performance improvement program as it relates to wound care including identification of trends and rates of wounds throughout the facility, compliance with all wound care, care delivery.
Compliance : Ensure compliance with all federal, state, and local health regulations.
Nursing : May be required to intermittently work shifts as a medication nurse and / or Supervisor as needed when there is a shortage.
SKILLS AND COMPETENTINCES :
Proficient in conducting basic clinical assessments and administering vaccines. Excellent organizational skills with attention to detail and record-keeping accuracy. Strong interpersonal and communication skills, with the ability to interact effectively with employees at all levels. Ability to handle sensitive health information confidentially and professionally. Competent in using electronic health record (EHR) systems and standard office software.
