DESCRIPTION OF WORK

In this role, you will be responsible for performing duties in support of the Pennsylvania Caregiver Support Program, which links care giving adults with support and financial reimbursement. Work involves serving as an OPTIONS Aging Care Manager, where you will work closely with other social agencies, hospitals, clinics, and community resources to develop plans in order to meet the needs of clients and assist them in utilizing these resources. You will also have the opportunity complete assessments by obtaining essential information to determine the needs of care receivers and establish avenues to meet them. Additional responsibilities include :

• Determining eligibility of services and supplies based on income
• Processing receipts for in-home services or supply reimbursement
• Tracking expenditure of funds
• Completing data entry for case notes, services orders, services deliveries, and more
• Performing assessments for meals on wheels, personal care, home support, adult day care, and referrals for other agencies
