Primary Care Physician - Full Time
Can be based in Cortez, Dove Creek, Durango, or Pagosa Springs, Colorado
Position Summary : The role of the Primary Care Physician is to provide medical services as established in the clinic care protocols. Participates and collaborates with multi-disciplinary teams to deliver comprehensive integrated healthcare services. Demonstrates advanced application of primary care and pharmacologic knowledge, skills, critical thinking, and analysis. Provides medical perspective and expertise in treatment and problem solving, decision making, and activating clinic resources to achieve patient outcomes. Consults with staff as necessary to instruct them on clinical interventions necessary for achieving quality patient outcomes.
Our ideal candidate for our Primary Care Physician position will be completing or will have completed an approved residency training program in family medicine and be certified or eligible for Board Certification in Family Medicine. Colorado licensed physician and current unrestricted DEA certificate required. Ideal candidate will have had at least three (3) years of professional clinic experience, including the participation in multidisciplinary teams in a primary care setting.
Salary : $200,012.80 - $212,014.40 annually and considers preferred experience and preferred education described below.

Physical demands : Employee must occasionally lift and / or move up to 15 pounds. Employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between Axis Health System locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. Additional eligibility requirements : Annual Flu immunization, Annual TB screening, CPI training within 90 days of hire, BLS PRO certification required within 90 days of hire.
