Southern Illinois Healthcare is seeking a collegial and compassionate Physician Assistant (PA) or Nurse Practitioner (NP) to join our Inpatient Rehabilitation Facility (IRF) team within the PM&R clinic. This role is ideal for an experienced provider passionate about rehabilitation medicine and collaborative care.
Position Highlights
- Manage daily medical needs of IRF patients, including rounding, medication management, and care coordination.
- Conduct comprehensive patient assessments, develop treatment plans, and monitor progress toward rehabilitation goals.
- Collaborate with an interdisciplinary team to support functional recovery and safe discharge planning.
- Perform and document history and physicals, progress notes, and discharge summaries per regulatory standards.
- Participate in admissions and evaluations of IRF candidates to ensure medical appropriateness.
- Provide consultative services in the acute care hospital for patients requiring physiatry input.
- Communicate effectively with physicians, nursing staff, and therapists to ensure seamless transitions of care.
- Engage in quality improvement initiatives, team meetings, and compliance reviews.
Qualifications
Licensed Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in Illinois.Certification as a Nurse Practitioner or Physician Assistant (e.g., FNP, AGACNP, or equivalent).Minimum 2 years of experience in acute care, rehabilitation medicine, internal medicine, or a related field preferred.Strong clinical assessment, documentation, and communication skills.Proven ability to work collaboratively within an interdisciplinary team.Schedule
Full-time position with primary coverage at the IRF.May include consults at the acute care hospital.Weekend and holiday rotation required to ensure continuity of patient care.
