As a Nurse Practitioner You will :
Provide compassionate medical care and symptom managementPerform assessments, order / interpret diagnostic tests, and develop care plansPrescribe medications, manage treatments, and coordinate hospice servicesConduct and document face-to-face visits for eligibility and recertificationCollaborate with physicians and IDT team to align care with patient goals.Participate in care planning and coordinate with team and external providersMaintain accurate clinical documentation and ensure compliance regulationEducate patients & families on medical, emotional, and spiritual needsContribute to quality improvement efforts and professional developmentServe as a clinical resource and liaison to community partnersResponsibilities

Graduate of an accredited School of Nursing and Nurse Practitioner programMaster’s degree with a minimum of one year NP experienceOne year of NP experience or one year of RN hospice experienceMinimum of one (1) year hospice or palliative care experience preferredCurrent nursing license in the state of practiceCurrent DEA license (if applicable to state regulations)Current CPR certificationCertification from an approved advanced practice nursing certifying body, as required by the stateNurse Practitioner Certification in Gerontology, Adult, or Family (preferred)Excellent observation, verbal, and written communication skillsStrong problem-solving and mathematical skills
