Family Practice / Primary Care Physician Assistant

Chart closure within 72 hours

Schedule = 18 PPD, minimum is 18 PPD

Monday-Friday 8-5, open to 10 hour shifts

Mix of peds / adults

Must have at least 3 years experience in similar setting (panel management)

Needs experience with Chronic Pain Management

BE will be considered if truly BE (5 years or less since residency)

Active DEA needed before start

Credentialing takes 45-60 days

The following immunizations are required for credentialing (before start date) : Flu Vaccine, TB Status / X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years),
