We are seeking an experienced, reliable Home Health Visit Nurse (PRN or Part Time) to provide targeted care and nursing services, with occasional recertifications, and supervisory visits (RNs only), to our clients across 5-7 counties in the Cleveland + upper NorthEast areas of Ohio.
- Position : Home Health Visit Nurse (Full Time)
- Location : Based anywhere with ability to serve 5-7 counties in the Cleveland + upper NorthEast areas of Ohio, including Cuyahoga, Lorain, Lake, Medina, and Geauga counties [+ Summit and Portage counties if able]
- Type : PRN (as needed) or Part Time
- Hours : As needed on weekdays only. No weekends or on-call shifts.
- Requirements & Qualifications :
Active RN preferred / LPNs with home health agency experience will be considered, with active license and ability to practice in the State of Ohio.
Minimum of 1 year experience as a paid nurse; 3+ years preferred .1+ years with a home health agency or equivalent in-home care environment is preferred but not required.Active CPR and First Aid Certification, or willingness to renew prior to start.Strong background in nursing, patient care, safety, compliance, and confidentiality.Desirable character traits : reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.Current Driver's License and reliable transportation.Other Details and Perks :
This is a PRN or Part Time position with hourly pay as needed, with flexible, customizable schedules, and reporting to experienced RNs, not non-clinical business managers. As a Home Health Visit Nurse , you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.
Following a physician-established plan of treatment and working under the supervision of the Director of Nursing, our Home Health Visit Nurses instruct patients and their families in proper treatment and help individuals and groups take steps to improve or maintain health.With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his / her family.Our nurses function as a Caregiver, Teacher, and Advocate; working in partnership with the family, our nursing staff brings a commitment to support and assist with the family in their caregiver role. As a caregiver, the nurse will provide ongoing assessment of our patient's condition response to treatments and developmental progress. As a teacher, the nurse will instruct the family on procedures, medications, safe use of equipment, and compliance with the treatments.
