Unlock Your Full Potential as a Case Manager
Collaborate with an interdisciplinary team to adjust treatment plans based on patient needs and preferences.Establish tentative discharge plans and contingency plans with the team.Oversee the patient discharge experience and ensure satisfaction.Monitor patient metrics such as quality, service appropriateness, and compliance.Facilitate weekly team conferences and coordinate modifications to treatment plans.Complete necessary documentation and case management addendums accurately.Stay informed about regulations, company policies, and department operations.Analyze case management reports and determine necessary actions.Attend meetings to identify trends and collaboratively reduce Acute Care Transfers (ACT).Meet with patients and families promptly upon admission to build rapport and establish communication.Conduct assessments and complete necessary documentation within established timelines.Educate patients and families about the rehabilitation process and case management roles.Organize family conferences as needed to support patient care.Assist patients in planning and procuring needed resources to prevent discharge delays.Ensure compliance with regulations surrounding orthotics and prosthetics.Make timely referrals to post-discharge providers as required.Verify accuracy of discharge and billing information in patient records.Participate in the utilization review process to enhance care outcomes.Take part in the on-call schedule for case management support as needed.Required Qualifications
License or Certification :Must independently perform assessments within your discipline's scope (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling). Licensure must be active if required by the state.Eligibility for CCM® or ACM certification is required upon entering the position or within two years of starting.CCM® or ACM certification must be obtained within two years for Case Manager II positions.Minimum Qualifications :
For Nursing, an Associate Degree in Nursing is required, with RN licensure (BSN preferred). A diploma is acceptable in states that recognize it as the minimum licensing requirement.For other licensed or certified healthcare professionals, a minimum of a bachelor's degree is required, with a graduate degree preferred.A minimum of two years of rehabilitation experience is preferred.The Encompass Health Philosophy
