Job descriptionProvide compassionate home-based care to patients recovering from illnesses, injuries, or chronic diseases. Collaborate with a team of physicians and oversee LPNs, CNAs, and aides in delivering patient care. Conduct patient assessments, document progress, and communicate changes to physicians. Educate patients and their families on care plans, medications, and home safety. Ensure patient goals are met and coordinate discharge from services. A collaborative leadership team that values every member’s contribution. Hardworking clinicians and supportive office staff united in providing the best care. A strong emphasis on maintaining a healthy work-life balance. An active, unrestricted Registered Nurse (RN) license in the State of Michigan (BSN not required). At least one year of clinical experience as a Registered Nurse. Current CPR Certification. Reliable transportation, a valid state driver's license, and current car insurance.
