Home Health Nurse

Job Description

Key Responsibilities :

Conduct comprehensive patient assessments in the home setting.

Develop and implement individualized care plans based on physician orders and patient needs.

Administer medications, wound care, IV therapy, and other nursing procedures.

Monitor patient progress and update care plans accordingly.

Qualifications :

Licensure : Active RN license in the state of practice.

Experience : 1+ years of clinical nursing experience (home health preferred).

Certifications : BLS (Basic Life Support) required.

Skills :

Strong clinical judgment and critical thinking.

Excellent communication and interpersonal skills.

FLU is mandatory during FLU season. No religious or medical exemptions will be accepted.

• Offered pay rate will be based on education, experience, and healthcare credentials.
