An established, state-of-the-art, award-winning patient care facility is seeking a dedicated Physician Assistant to join their team! This practice emphasizes work/life balance while delivering top-tier care to a geriatric patient population in an outpatient setting. We're seeking a provider who is bilingual in English and Spanish with a strong commitment to preventive care and evidence-based medicine.

• Base salary: $120,000 - $130,000, with 10%-15% performance-based bonuses

Responsibilities

• Average 10 to 15 patients per day
• Focus on chronic disease management
• Develop strategies to reduce ER visits and hospitalizations
• Collaborate with Case Managers and Specialists
• Educate patients on preventive care and treatment adherence

Requirements

• Experience in value-based care - Required
• Licensed PA in the state of Florida - Required
• Fluent in English and Spanish - Required
• Experience with eClinicalWorks (EMR) - Preferred
• Passion for caring for a senior population
