Shift Differentials :

Monday - Friday Evening / Night : $2.00

Weekend Day : $2.75

Weekend Evening / Night : $4.75

Position Overview :

The primary responsibility of the Licensed Practical Nurse (LPN) is to plan, organize, and provide nursing care to residents according to federal, state and facility regulations and standards and ensures quality care is maintained.

Essential Job Functions :

LPNs provides professional nursing care to residents :

LPNs observe residents' conditions and charts and report changes or concerns to the appropriate parties :

LPNs respond to residents' call for help and determine how to assist them :

LPNs work with DON, physicians, Care Plan Team, nursing staff and residents' families to assess residents' needs, plan and modify care and implement interventions :

LPNs help residents with activities of daily living (ADLS) and ensure that residents are following diets as directed by physicians :

LPNs maintain work areas in a clean and orderly fashion :

Physical Requirements :

Medium Work - Exerting 20 to 50 pounds of force occasionally, and / or 10 to 25 pounds of force frequently and / or greater than negligible up to 10 pounds of force constantly.
