What We Offer : Work Schedule - 8 am - 5 pm Status - Non-Exempt Use of Company Car and Gas Card to / from patient homes to conduct assessments. 1. Patient Assessment and Care Planning Conduct comprehensive initial and ongoing patient assessments in the home environment. Collect and accurately document patient data according to OASIS requirements. Identify patient needs, risks, and potential complications. Develop, implement, and update individualized care plans based on assessment findings. Collaborate with patients, families, and interdisciplinary teams to ensure holistic care. 2. OASIS Data Management Complete all OASIS assessments timely and accurately per regulatory guidelines. Ensure data quality and integrity for OASIS submissions to meet compliance standards. Analyze OASIS data to identify patient outcomes, needs, and service effectiveness. Use OASIS data to support reimbursement, quality reporting, and clinical decision-making. 3. Care Coordination and Communication Coordinate care with physicians, therapists, social workers, and other healthcare providers. Facilitate timely referrals and transitions of care. Communicate effectively with patients and families regarding care plans, treatment goals, and patient education. Serve as the primary point of contact for patients and caregivers throughout the home health episode. What We're Looking For : Education : 2 Year / Associate Degree, required. Associates Degree in Nursing or BSN. 4 Year / Bachelors Degree, preferred. Experience : Two or more years healthcare experience, preferred. Case Management or related experience, preferred. Licensure / Certification : Current RN licensure in state of employment, required. Additional skills required : Weekend and holiday coverage, if applicable. Additional skills preferred : Knowledge / Competence of Microsoft office products and keyboarding.
