Location : Tallahassee, FL

Schedule : Full-Time - Day Shift (8 : 00 AM-4 : 30 PM) or Evening Shift (12 : 00 PM-8 : 30 PM)

Weekend Requirement : 1 weekend per 4-week schedule

Pay Range : $31.94 - $43.92 per hour (based on experience)

Differentials : $2.50 / hr for evenings | $2.00 / hr for weekends | Stackable : $4.50 / hr if working both

Sign-On Bonus : $10,000 paid in 5 installments over 2 years

Relocation Assistance : Case-by-case basis

Overview :

Join the dynamic case management team at a 288-bed acute-care hospital serving Leon and Gadsden Counties. As a Case Manager RN , you will facilitate high-quality patient care by coordinating interdisciplinary treatment plans and discharge processes to ensure efficient resource utilization and excellent patient outcomes.

Responsibilities :

• Perform assessments of psychosocial and medical needs
• Develop and coordinate interdisciplinary care plans
• Lead and participate in case reviews and team meetings
• Monitor medical necessity and admission criteria
• Communicate with third-party payers and facilitate transitions of care
• Advocate for patients across the continuum of care
• Identify care barriers and develop action plans for improvement

Requirements :

(OR)

3 years of combined experience in Med / Surg, Tele, ICU, PCU, Neuro, or ED

(OR)

Home Health / Insurance case management experience with at least 1 year acute care experience within the last 5 years
