AMG Specialty Hospital- Albuquerque seeks a Full-Time CaseManager (RN). The Case Manager'sprimary function is to make readily available to the patient, their familiesand / or significant others, special services in order to facilitate adjustmentof these individual's continuity of care post discharge. The Case Manageroversees the effective coordination of admission and discharge planning, casecommunications, interdisciplinary team conferences, insurance relations andassurance through managed care systems, and verbal and written reporting. Assures regulations regarding patient rights are fulfilled. Allcandidates must have a Registered Nurse (RN), New Mexico License in good standing.

• Minimumof six months to one year of discharge planning experience preferred.
• Abilityto read and communicate effectively in English.
• Additionallanguages desirable.
• Excellenttime management skills; organized and able to prioritize
• Proficientcomputer skills.
• Possessessuperb written and spoken communicationskills
• Excellentinterpersonal skills with colleagues and others
• Comfortablein a fast-paced environment with multiple cases
• Ableto organize and manage large amounts of files, schedules, dates, and information
• Self-directedand able to work without supervision
