RN CASE MANAGER (HOSPICE) | DETAILS AND COMPENSATION :

Location :

Colonial Heights VA 23834 – 100% On-site / Fieldwork

and surrounding areas (Chesterfield, Tri-Cities, Hopewell, Petersburg)

Payrate : $56.14 / hr

Required Availability :

Full-Time | Monday–Friday, 8 : 00 AM–4 : 30 PM

(some on-call required as needed)

The

RN Case Manager

role focuses on providing holistic, patient-centered care coordination and ensuring continuity across settings while supporting patients and families through their healthcare journey.

This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
