• The RN Case Manager (RNCM) is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end-of-life care.
• In this role, the RNCM collaboratively works alongside an i nterdisciplinary team to provide end-of-life care to those needing hospice services in their home or community setting.
• The RNCM provides individualized case management, admission facilitation, clinical support, continuous care, after hours call, and / or other skilled nursing expertise with chronic and end-of-life care.
