Job Summary and Qualifications

As a Registered Nurse Care Manager, you will provide coordinated skilled nursing care to patients of all ages in their homes. You will collaborate with an interdisciplinary team to establish patient goals, ensuring high-quality care under the supervision of the Clinical Manager.
