Washington Licensed Clinical Program Coordinator RN

A company is looking for a Clinical Program Coordinator RN, Medicare / Medicaid who will provide care coordination and management services for members.

Key Responsibilities

Provide care coordination, case management, and care management services to members

Educate and empower members to manage their diseases through disease management programs

Assist with managing member health plan benefits and improve access and quality of care

Required Qualifications

5+ years of clinical nursing experience

Experience collaborating with physicians in patient care management

Current unencumbered Registered Nurse License in state of residency
