• Required Skills : Facilitate discharge and contingency planning,Educates patient / caregiver to facilitate discharge,Suicide / homicide / self-harm,Psychiatric Disorders,High risk intervention patient vulnerability or safety risk,Knowledge of cultural issues,NASW Code of Ethics / Standards of Home Health Care,Complete intake and admission screenings,Participate in development of treatment plan,Perform Bio-psychosocial assessments,Provide patient and family education, support and advocacy,Work with multi-disciplinary team to ensure continuity of care
