Your Responsibilities :

Provide nursing care to residents with diverse needs, including therapeutic, neurological, dementia, dialysis, IV therapy, infectious disease, and wound care.

Administer medications and treatments as prescribed, ensuring proper documentation and resident safety.

Monitor residents’ conditions and collaborate with the Charge Nurse and interdisciplinary team to address care needs.

Support residents’ physical, emotional, and psychosocial well-being.

Assist in implementing and following individualized Care Plans.

Maintain compliance with facility policies, state regulations, and resident care standards.

Perform additional duties as assigned to meet resident and facility needs.

Qualifications :

Graduate of an accredited nursing program.

Active LPN license  in Pennsylvania, in good standing.

Strong clinical, communication, and organizational skills.

Geriatrics

Work Environment :

Skilled nursing facility in  York, PA  (placement through our agency).
