Your Role : Shaping Exceptional Care

As a Nurse Manager, you’ll be a critical leader in ensuring our residents receive the highest quality of care. Your key responsibilities include :

• MDS Assessments :  Complete comprehensive and accurate MDS assessments in compliance with federal and state regulations.
• Resident Care :  Conduct thorough assessments of residents’ physical, psychological, and social needs, utilizing the MDS tool to document their status effectively.
• Care Planning :  Develop and maintain individualized care plans based on assessment data. Lead interdisciplinary care plan meetings to ensure collaborative and effective care strategies.
• Compliance Excellence :  Ensure all practices meet CMS guidelines, state regulations, and industry standards.
• Team Collaboration :  Partner with physicians, therapists, dietitians, social workers, and other healthcare professionals to provide holistic and coordinated care for residents.
