Hourly Pay Range :

$38.16 - $59.15 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights :

• Position : RN Case Manager
• Location : Swedish Hospital, Chicago. IL
• Full Time
• Hours : Monday-Friday, Flex schedule AM / PM based on need. AM : 7 am - 3 : 30 pm, PM : 3 pm - 11 : 30 pm. Rotating weekends, rotating holidays (Expectation is 1 summer holiday and 1 winter holiday)
• Required Travel : Supporting the Emergency Department at Swedish Hospital, but will train at Glenbrook Hospital and float to Glenbrook, as needed.

A Brief Overview :

The RN Case Manager plays a critical role in ensuring the efficient and effective coordination of patient care. This position involves working closely with interdisciplinary healthcare teams to assess, plan, implement, monitor, and evaluate the healthcare services provided to patients. The RN Case Manager helps optimize patient outcomes, improve resource utilization, and enhance the overall quality of care.
