Job summary:
The Permanency Programs Social Worker (PPSW) will provide cross-over functions in the following areas: Adoption, Foster Care, Intensive Services Foster Care (ISFC), Therapeutic Foster Care (TFC) and Transitional Housing. The PPSW will conduct home environment assessment, psychosocial assessment, and prepare a Written Report for resource families. This position will: develop and coordinate needs and services plans, conduct home-based case management services for resource families; assess and identify family needs and connect families to available resources. Participate in Child and Family Teams (CFT) meetings and write quarterly reports on child’s progress. Provide case management to assigned Transitional Aged Youth (TAY) in our Transitional Housing Program + Foster Care. This includes assisting program participants with mental health needs, vocational, and educational needs. The PPSW will be active in implementing and supporting a strengths-based, culturally competent, participant-centered approach to working with TAY.

Position Type/Expected Hours of Work
This is a part-time position with a wide availability of hours, times and days. An applicant with a flexible schedule is highly desirable. (Hours are subject to change to meet business needs).
Travel
Local travel to various worksites is required.
Work Authorization/Security Clearance
Employees must pass LiveScan check and receive TB and health clearances prior to employment. Covid-19 vaccination is required.
