Base Pay : $66,560.00 - $86,528.00
JOB SUMMARY
The Supportive Services Center Manager oversees the day-to-day operations and supervises the case management staff directly for the Enhanced Patient Services Team and Medical Respite Team as well as the support staff at the Juvenile Rehabilitation Facility. The Supportive Services Center Manager implements and communicates procedures and systems to the case management staff consistent with the procedures and systems of SCHC and social work case management standards. The Supportive Services Center Manager participates in the planning, shaping, and administration of the staff while keeping the mission, vision, and values of the organization in mind always. The Supportive Services Center Manager works closely with Medical Director, HOPE Senior Program Director, or Senior Director of Clinical Operations to address any issues in a timely manner for all programs under their supervision. In addition, the Supportive Services Center Manager exercises a high degree of independent judgement under minimal supervision. The Supportive Services Center Manager empowers staff for excellence in creating an optimal customer experience through our positive workplace policy.
JOB DUTIES AND RESPONSIBILITIES
Management, Supervision, and Training
Coordinates, delegates, and oversees activities associated with the day-to-day case management operations including :
- Referral assignment and coordination, including management of any waitlists.
- Ensures smooth operation of the case management programs during all hours of business.
- Facilitates orientation and ongoing training and support of case management and clinical staff when pertinent to their respective roles and clinical staff when pertinent to their respective roles
- Has familiarity with all grant requirements of the case management program, Juvenile Justice and Medical Respite.
- Provides supervision of direct clinical and social service staff for the Case Management programs, Juvenile Rehabilitation Facility, Medical Respite and Street Medicine programs.
- Assists in the recruitment of staff that report to the programs under their supervision.
- Oversees the day-to-day operations of hotel stays for patients approved by SCHC staff under their supervision.
- Able to provide case management services for complex and highly acute patients.
- Able to remain current on laws and regulations that impact clinical service delivery in incarcerated settings for youth.
- Fosters a team atmosphere with all staff and consistent with the PCMH model.
- Cites and exemplifies the mission, vision, and values of SCHC.
- Cites and exemplifies excellent customer service in all interactions with patients and employees.
- Handles patient complaints daily.
- Maintains consistent communication regarding policies, procedures, and protocols among all staff.
- Works with SCHC Informatics Department to support consistent data utilization.
- Oversees the billing and coding process of Cal Aim, including but not limited to TARS, QIP efforts and other Partnership / DHCS required reporting.
- Maintains a culture of accountability and consistency with staff in respective clinic.
- Works with Medical Director and HOPE Senior Program Director to ensure productivity goals are achieved.
- Works closely with staff to monitor and meet quality indicators and objectives.
- Ensures compliance with finance / billing processes to meet goals and objectives (e.g., accurate collection of data, timely submission of data).
- Work with the HOPE Senior Program Director and the Grants department to ensure compliance, goals and objectives are met with for any grant applications.
- In conjunction with the HOPE Senior Program Director, collects and provides important data points and deliverables associated with grants related to the programs they manage.
- Ensures timecard completion.
- Ensures grant time sheet completion.
- Participates and exhibits competency in management training series—classes will include subjects in leadership, HR, finance and billing, operations, information systems, clinical skills, EHR navigation, report extrapolation,
- Harm Reduction, Housing First, Trauma Informed, Pretreatment principles, outreach safety, Housing First, joint departmental meetings.
- Meets technical competency standards as required for all staff.
- Works directly with employees and patients at least two days per month to ensure firm understanding of systems and processes from the staff point of view.
- Work with the HOPE and JRF clinical team to ensure that there is consistent communication regarding case management service delivery.
- Work closely with the Clinical HOPE Center Manager to make sure there is clear communication and staff support.
- Could include adjusting staffing levels up or down depending on daily needs and funding streams.
- An ability to understand and utilize the community resources and structures that support Homeless Outreach and Resource coordination.
Operations
Participates in various committees on a rotating basis.Collaborates with the HOPE Senior Program Director and Medical Director of HOPE regarding training and updates to policies and procedures as they relate to the clinic.Participate in joint Departmental trainings.Ensures financial viability of health center by displaying a firm understanding of the budget process and enforcing compliance with necessary finance / billing processes.Actively setting and managing department budget on an annual basis to help meet organizational budgetary goals.Coordinate with the Clinical HOPE Center Manager about department budget needs.Administrative
Performs other duties, projects as requested by the HOPE Senior Program Director and the HOPE Medical Director.Leads and attends Clinic / Center-wide meetings as required.KNOWLEDGE, SKILLS, AND ABILITIES
Demonstrates high level of social work skill set and case management knowledge.Demonstrate skill in communicating effectively both orally and in writing.Ability to take directions and complete tasks on time.Good organization skills.Strong EHR navigation and report extrapolation experience.Understanding of generally accepted accounting principles.Experience and working knowledge of Excel, MS Word, PowerPoint.EDUCATION & EXPERIENCE
Bachelor’s Degree in related field, or equivalent (e.g., Registered Nurse).Previous experience in related field.Excellent people skills and customer service orientation.SCHC is an Equal Opportunity Employer
