LPN - Team Lead AuthoraCare House Calls

Brief Description

AuthoraCare Collective is currently seeking an LPN – Team Lead AuthoraCare House Calls (Primary Care). This is a Full-time position : Monday – Friday 8 : 00am – 5 : 00pm. Office based in High Point.

CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our primary care team. This position is primarily responsible for telephonic patient case management within the Chronic Care Management (CCM) / Principal Care Management (PCM) scope. The LPN collaborates with physicians, nurse practitioners, and interdisciplinary team members to provide high‑quality, patient‑centered care.

As the LPN Team Lead, you will play a critical role in supporting a high‑functioning interdisciplinary team delivering in‑home primary care to complex, homebound patients. This position is responsible for overseeing daily clinical workflows, ensuring timely documentation and coordination in Athena EMR, managing visit schedules and field staff operations, and promoting operational efficiency.

This role requires sound clinical judgment, strong communication skills, and the ability to support patients with complex medical needs. Requirements

Graduate of an accredited LPN program.

Current North Carolina LPN license.

Experience in primary care, case management, or chronic care preferred.

CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Valid state‑issued driver’s license required. Must carry automobile liability insurance at limits required by agency and have own transportation.
