Licensed Clinical Social Worker (LICSW) - Case Manager
Location : Washington, D.C. Area (On-Site / Field-Based Role)
Schedule : Full-Time, Monday–Friday (8 : 00 AM – 5 : 00 PM)
Compensation : Competitive Hourly Rate Based on Experience
Overview
Join our team as an LICSW Care Manager and play a vital role in coordinating care for Medicaid members, including children, who face complex health and socio-economic barriers. You will work to actively break down barriers for enrollees and their families, utilizing your clinical skills to manage a diverse caseload and optimize health outcomes. This role offers a supportive training environment and the opportunity to work collaboratively with a major academic health system.
Key Responsibilities
- Caseload Management : Conduct initial needs assessments to assign patient acuity levels (1–3) and manage a standard caseload of approximately 75 individuals (mixed acuity) or a specialized caseload of 60 complex Level 3 acuity individuals .
- Field-Based Care : Complete a minimum of three field visits per week for care plans or assessments, using your judgment to schedule these visits flexibly.
- Care Coordination : Develop and implement effective care plans, facilitate well-child visits, and build strong relationships with primary care providers to optimize member support.
- Documentation & Compliance : Process prescription data, maintain confidentiality, and ensure compliance with all Medicaid contractual obligations and documentation standards.
- Intervention : Work with enrollees and families to address socio-economic challenges that impact health and break down barriers to care.
Qualifications
Must-Have Skills / Qualifications :
LICSW licensure from Washington DC is required.Minimum of two years of experience in a clinical setting.Background in pediatric, health plan, or behavioral health required.Preferred Qualifications :
Five or more years of experience is highly valued.Direct experience working with Medicaid members or within a health plan environment.Soft Skills Required : Articulate communication, empathy, organizational skills, and a strong desire to serve the underserved population.
