Florida Licensed Care Manager RN

A company is looking for a Care Manager (RN) to support Medicaid services in Florida.

Key Responsibilities

Evaluate member needs and develop personalized care plans to improve healthcare outcomes

Coordinate with providers and community resources to address members' unmet needs

Monitor member progress and adjust care plans as necessary to meet goals

Required Qualifications

Active Registered Nurse (RN) license in Florida

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in case management, discharge planning, or acute care

Ability to thrive in a fast-paced environment

Strong organizational skills
