Setting : Primary Care Outpatient; Mon-Fri (no weekends)

Position Summary :
The Licensed Practical Nurse (LPN) is an essential part of our care delivery team, supporting physicians, nurse practitioners, and registered nurses in delivering comprehensive and respectful care. In this role, you will provide clinical and administrative support to ensure patients—many of whom face social, economic, and linguistic challenges—receive timely and effective treatment.
Key Responsibilities :
Clinical Duties :
- Take vital signs and conduct routine screenings during patient intake.
- Administer medications and immunizations under provider or RN supervision.
- Perform basic wound care, EKGs, and other clinical procedures as appropriate.
- Support providers during examinations and minor procedures.
- Monitor and document patient status and report significant findings to the RN or provider.
Patient Engagement & Advocacy :
Provide clear and compassionate education on treatments, medications, and self-care.Build trust and rapport with patients from diverse backgrounds.Assist patients with follow-up appointments, referrals, and resource navigation.Administrative Support :
Maintain accurate, up-to-date records in the electronic health record (EHR).Ensure clinical supplies are stocked and equipment is sanitized.Participate in quality and safety initiatives and team meetings.Qualifications :
Current LPN license in MarylandGraduate of an accredited Practical Nursing programCurrent BLS certification requiredPrevious experience in a primary care, community health, or outpatient setting preferredBilingual or multilingual candidates are strongly encouraged to applyExperience working with diverse or underserved populations is highly valuedIdeal Candidate Will Be :
