Job Title : Physical Therapist Assistant (PTA) – Home Care

Location : Mansfield, CT

Division : Home Care

Shift :

  • Monday–Friday
  • 8 : 00 AM – 4 : 30 PM

Position Overview

Seeking a dedicated Physical Therapist Assistant (PTA) to join our Home Care division. This role involves providing high-quality rehabilitative care to patients in their homes, supporting their recovery and independence.

Key Responsibilities

  • Deliver physical therapy services under the supervision of a licensed Physical Therapist.
  • Assist patients with therapeutic exercises and mobility training in a home care setting.
  • Monitor patient progress and report findings to the supervising therapist.
  • Ensure compliance with all state and facility regulations.
  • Maintain accurate documentation of care provided.

    • Requirements

  • Experience : Minimum of 1 year in home care preferred.
  • Licenses / Certifications : Active Physical Therapist Assistant (PTA) license .
  • Connecticut state license (must be obtained prior to start).
  • Other : Reliable transportation for home visits.

  • On-Call : Not required.
  • Expected Ratios / Cases : TBD.
  • Start Date : Upon completion of credentialing and licensure verification.
