Description :
This position requires a Registered Nurse specializing in case management.
The primary responsibilities include providing and coordinating medical home care, placement, and social services.
The role focuses on ensuring safe patient discharges.
The average daily patient case load is 15 cases.
Candidates should have a minimum of 2 years of relevant experience.
First-time travelers are welcome to apply.
Experience with electronic health record systems is preferred.
Candidates must be prepared to work holidays as needed.
Two satisfactory professional references are required, preferably from recent employers.
Candidates should be able to work in a fast-paced environment and demonstrate strong organizational skills.
Comfort in floating to other departments as necessary is required.
