Licensed Vocational Nurse (LVN)

Reports To : Client Services Operations Manager

Department : Mosaic R-10

Positions Supervised : DSP Leads

DEFINITION : The Licensed Vocational Nurse (LVN) provides nursing assistance to clients at the Mosaic Day Program under the supervision of the program’s Registered Nurse and in conjunction with the Program Coordinator.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

Nutrition and hydration via G-tubes, including :

Care of equipment required for feeding and hydration

Proper storage of equipment in a sanitary place

Maintenance of records for each client fed, including formula type, amount, and hydration.

Follow all guidelines per medical doctor and health care protocols.

Administer and record prescribed medications via G-tubes as required.

Administer prescribed medications (oral, injection, inhaler, drops, creams, etc)

Assist with procedures as outlined in the licensing guidelines regarding Title 22, Restricted Health Conditions §82092, which includes :

Inhalation assistive devices

Decubitus ulcers / pressure areas

Positioning

Infections

Colostomy / Ileostomy care

Catheter care

Insulin-dependent Diabetes

Wounds

Gastrostomies and tracheostomies

Assist with medical records required on admission to the program and subsequent follow-ups.

Communicate with care home staff / families regarding contagious diseases, medical concerns, and other medical / health care issues as they arise in the Adult Day Programs.

Educate and monitor staff regarding skin care, positioning, hydration, medication administration, and general health and safety.

Actively participates in staff meetings & client conferences.

Attends required in-service training and implements training received.

Keeps immediate supervisor promptly and fully informed of all problems or unusual matters of significance and takes prompt corrective action where necessary.

Performs other duties and responsibilities as required or requested by the Program Coordinator, Client Services Operations Manager, and the Director of Client Services.

PHYSICAL DEMANDS : The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

Employees are regularly required to stand, walk, bend, lift, use hands, reach with hands and arms. The employee is occasionally required to sit for extended periods of time.

Specific vision abilities required by this job might include distance vision, color vision, peripheral vision, depth perception, and the ability to focus.

Employees must be able to operate a lift and transfer people being served from wheelchair to chair and provide physical assistance as necessary in the performance of the job.

QUALIFICATION REQUIREMENTS : The requirements listed below are representative of the knowledge, skill, and / or ability required to successfully perform this job.

REQUIRE :

Must be 18 years or older

California license as a Licensed Vocational Nurse (LVN)

Current First Aid and CPR certificate. Required to maintain current certification as a condition of continued employment.

The ability to work as a team member.

The ability to represent the Work Training Center in a positive manner.

A pre-employment health screen, negative TB screening, and FBI fingerprint clearance from the Department of Justice on file as a condition of continued employment.

Must pass pre-employment physical ability testing and drug screen analysis.

Ability and willingness to work overtime as required.

Adherence to established dress standards.

PREFER :

Experience and / or training in human services or a related field providing direct services to persons receiving services with developmental disabilities.

Experience and ability to implement non-aversive positive behavior management techniques.

It is recommended that staff be tested and inoculated for Hepatitis B.

Monday - Friday 8 : 00 a.m. - 4 : 00 p.m.
