Job Description

Responsible for counseling, assessing, and assisting individuals, families, or groups affected by illnesses or injuries, including rape, physical abuse, cancer, AIDS, and Alzheimer's.

Principal Accountabilities

• Provides psychosocial assessments to determine patient needs; makes appropriate referrals; provides counseling and supportive services to patients / families; makes referrals to the Division of Child Protection and Permanency (DCP&P).
• Identifies and maintains funding sources that support the Family Counseling & Guidance Program.
• Maintains statistical information and analysis of data; develops outcome measures for program evaluation, development, and facilitation.
• Other relevant duties as assigned.
