About the Assignment

HumanEdge Health is seeking a compassionate and independent Travel Registered Nurse (RN) for a Home Health and Hospice assignment in Littleton, New Hampshire . As part of a dynamic, multidisciplinary team , you’ll develop and implement individualized care plans while working with a high degree of autonomy and clinical judgment. This is an excellent opportunity for RNs who value flexibility, patient relationships, and holistic care.

Assignment Details

Schedule : Monday through Friday, 8-hour day shifts

Hours : 40 hours per week guaranteed

Weekends / On-Call : None required

Care Setting : Home Health and Hospice (community-based)

Work Structure : Flexible start / end times; option to begin and end day from home

Responsibilities

Develop and manage individualized care plans for Home Health and Hospice patients

Provide skilled nursing care in accordance with physician orders and agency protocols

Conduct patient assessments, administer medications, and perform wound care

Educate patients and families on health management and end-of-life care

Collaborate closely with interdisciplinary teams including therapists, social workers, and aides

Maintain accurate and timely documentation in the electronic medical record system

Qualifications

License : Active New Hampshire RN license (required)

Certifications : BLS (AHA) – required

Experience :

Minimum 2–5 years of Med-Surg experience – required

Home Health and / or Hospice experience – preferred

Skills & Attributes :

Self-directed, adaptable, and able to work independently

Strong interpersonal and communication skills

Comfortable functioning in diverse home environments

Computer proficiency with EMR systems
