Job Description
The Wound Care Nurse Practitioner (NP) reports to the CMO and supervises Nurses & Medical Assistants.
- Provides specialized wound care in home, nursing home, and assisted living settings for patients with chronic, pressure, surgical, and complex skin wounds.
- Performs assessments, develops care plans, educates patients / caregivers, and collaborates with healthcare teams to promote healing.
- Requires a Master’s or Doctoral degree in Nursing, current NP license, wound care certification within 6 months, and at least 2 years of relevant experience.
- Must have physical ability for fieldwork, including lifting and mobility, and reliable transportation with a valid driver’s license.
- Position involves travel, exposure to biohazards, and adherence to safety protocols.
- Key responsibilities include wound assessment, treatment, supervision of staff, documentation, and coordination of care.
- Strong assessment, communication, and adaptability skills are essential for success.
