Description:
As a continuing care LPN, providing top-notch patient care is paramount.
Under the direction of an RN, mid-level Provider, or Physician, you'll be responsible for assessing, planning, implementing, and evaluating patient care.
You'll be a vital part of our team, making a real difference in the lives of our patients and their families.
In this role, you will collect and monitor assessment data and report on patients' physiological, psychological, sociocultural, and spiritual needs.
You will provide direct patient care consistent with policies and procedures and unit-based routines.
You will inform patients and families about treatments, medications, equipment, and other related activities as appropriate.
Documentation of care will be performed in a manner consistent with established guidelines.
You will greet and communicate with patients and families in a caring and respectful manner.
Collaboration with other members of the health care team will be essential for continuity of care.
You will maintain a clean patient environment and communicate any equipment malfunctions.
Participation in selected quality monitoring activities under the direction of the healthcare team will be required.
High standards of moral and ethical behavior are expected in this position.
You will advocate for privacy, confidentiality, and security of patient and staff data.
Active participation in safety initiatives and completing all position-related safety competencies is required.
Other duties may be assigned as necessary.
