    • As an RN / LPN, Your Responsibilities Will Include :

  • Conducting ongoing assessments of the patient’s physical and psycho-social status
  • Collaborating with the Interdisciplinary Group (IDG), patient, family, and attending physician to develop a comprehensive care plan
  • Providing education and interventions to effectively manage pain and other symptoms
  • Ensuring the availability of necessary equipment, supplies, and medications
  • Making timely revisions to the IDG plan of care as patient needs evolve
  • Documenting patient and family services accurately and promptly
  • Regularly attending IDG meetings to stay aligned with team goals

    • What We’re Looking For :

  • Active, unrestricted RN / LPN license in the State of Michigan
  • At least one (1) year of relevant experience as an RN or LPN (required)
  • One (1) year of management experience (preferred)
  • One (1) year of home health experience (preferred)
  • Current CPR Certification
  • Experience in community health and a wide range of Medical-Surgical settings (preferred)
  • Reliable transportation with a valid state driver's license and current car insurance
