Description :
We are currently seeking a compassionate and detail-oriented Outpatient Care Coordinator to join our team in supporting individuals on their journey to recovery. This role is essential in facilitating smooth transitions of care, coordinating outpatient services, and ensuring a supportive, patient-centered experience. The ideal candidate will be a Certified Medical Assistant (CMA) or hold an equivalent certification , with experience or a strong interest in behavioral health and substance use treatment.
Duties / Responsibilities :
- Coordinate day-to-day scheduling and execution of medical services, including random drug screening schedules
- Conduct medical intake assessments and documentation for all new clients
- Participate in clinical team meetings for case discussions, treatment planning, and discharge coordination
- Serve as a liaison between clients, the medical team (ARNP, Medical Director), and clinical staff
- Coordinate with external partners such as pharmacies, laboratories, and pharmaceutical support programs
- Attend and contribute to leadership and performance improvement meetings
- Ensure timely and accurate EMR documentation in collaboration with the Compliance Coordinator
- Support development, implementation, and monitoring of medical-related policies and procedures
- Assist with maintaining licensing and accreditation standards under the direction of Compliance and Leadership
- Serve as the Infection Control Officer, maintaining and reporting on health and safety protocols
- Facilitate effective interdisciplinary communication to support integrated care
- Transcribe and submit provider orders to pharmacies as needed
- Provide education, support, and advocacy for clients managing complex or chronic health conditions
- Guide clients in navigating referrals, specialist care, and external treatment plans
- Collaborate with the ARNP to assess and monitor care quality and outcomes
- Review, interpret, and communicate lab results to the clinical team in a timely manner
- Ensure appropriate follow-up and confirmation for all abnormal drug screen results
- Monitor medication adherence, misuse, or signs of diversion, and medication counts
- Perform additional duties as assigned by the Clinical Director or Medical Director
