Job Description :

Sunbelt Staffing is hiring a Home Health RN in Aiken, SC. This role requires a nurse experienced in OASIS and proficient in Homecare Homebase (HCHB). The RN will perform field visits, deliver skilled nursing interventions, and ensure accurate documentation, supporting patients in achieving their health goals from the comfort of home.

Key Responsibilities :

Complete OASIS assessments for admissions, discharges, and recertifications

Provide skilled nursing care and education to patients and caregivers

Maintain accurate electronic documentation in Homecare Homebase (HCHB)

Coordinate patient care with providers and interdisciplinary teams

Adhere to Joint Commission and agency standards of practice

Qualifications :

Current RN license in South Carolina

2+ years of nursing experience, with 1 year in home health required

OASIS and Homecare Homebase (HCHB) proficiency mandatory

Reliable transportation, active drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
