• Your input in patient care is highly valued and encouraged\n\nYour Responsibilities as a Hospice Nurse : \n
• Conduct thorough initial assessments of patients' physical and psycho-social status, involving their families\n
• Collaborate with the Interdisciplinary Group (IDG), patients, and attending physicians to develop tailored care plans\n
• Educate patients and families on managing pain and other physical symptoms, while monitoring their responses to treatments\n
• Ensure necessary equipment, supplies, medications, and services are readily available for patient care\n
• Adapt the IDG care plan as patient and family needs evolve\n
• Document all patient and family services in a timely and accurate manner\n
• Participate actively in IDG meetings to ensure cohesive care\n\nWhat We Are Looking For : \n
• Registered Nurse with an active, unrestricted license in the State of Michigan\n
• Experience in community health and Medical-Surgical settings is preferred\n
