JOB TITLE : CASE MANAGER
DEPARTMENT : SERVICES
REPORTS TO : SENIOR CASE MANAGER
FLSA : NON-EXEMPT
LOCATION : JAMAICA, NYC
SALARY : $40,000.00 - $45,000.00
SUMMARY :
The Case Manager assesses clients’ needs, plans a course of action, links clients to services, and monitors the progress of clients and residents in their program. The Case Manager ensures that clients receive accurate and relevant information and referrals to assist them in obtaining and maintaining housing.
DUTIES & RESPONSIBILITIES :
EXPERIENCE, EDUCATION, AND SPECIAL REQUIREMENTS :
A bachelor's degree in social work or a related field, such as human services, is preferred. At least 1 year of case management experience working with the homeless, mentally ill, substance abuse, and / or low-income populations. Bilingual is a plus. Strong relationship-building skills are required. Ability to work independently, but know when to raise issues with the supervisor. Excellent organizational and time management skills are necessary. Ability to effectively multitask and follow up on a timely basis. Strong PC skills are required, including proficiency in Microsoft Word and Excel.
