    Responsibilities :

  • Assess patients to determine mental, physical, and spiritual needs.
  • Assess, plan, implement, and evaluate individualized care plans in collaboration with a multidisciplinary team.
  • Provide explanations, education, and instructions to patients and their responsible parties.
  • Use sound nursing judgment to recognize and report changes in patient condition.
  • Promote a healthy workplace by managing conflict with professionalism and clear communication.
  • Meet all core measure responsibilities related to patient care.
  • Participate in ongoing professional education.
  • Maintain complete and accurate medical records.
