- Attends work regularly according to assigned work schedule and in accordance with agency policy.\n
- Works cooperatively with others including all staff, supervisors, administrators, co-workers, individuals served, community professionals, customers, vendors and the public.\n
- Complies with the Delaware State Board of Nursing Practice Act.\n
- Completes a nursing ELP and establishes goals and objectives to address and meet the health care needs of each individual served upon admission and updated annually.\n
- Monitors the health status of each individual served every 30 days in accordance with the standards of DDDS.\n
- Within any Chimes Delaware contracted program assumes responsibility for screening program participants, rendering treatment, and determining the need for follow-up care.\n
- Checks physician orders against MAR.\n
- Provides hospitalization follow-up and post-hospitalization assessments, staff education, and care plan revision if warranted. Ensures follow-up is completed.\n
- Provides ongoing staff training as it relates to nursing / medical issues.\n
- Maintains required certifications that are necessary for compliance with training requirements.\n
- Administers the provision of medical and nursing services which are outside the scope of unlicensed staff or deemed inappropriate to be delegated to unlicensed personnel.\n
- Serves as a medical liaison when necessary.\n
- Coordinates medical care and appointments for individuals served through the appropriate health care facility.\n
- Assumes on call responsibilities.\n
- Responds to emergencies.\n
- Participates as a member of each individual's served interdisciplinary term and attend ELP meetings.\n
- Maintains confidentiality, in accordance with HIPPA, of all individuals served and promotes each individual's rights at all times.\n
- Contributes to a work atmosphere in a manner which is positive, enthusiastic, respectful and courteous.\n
- Is a positive role model for individuals served and Agency staff.\n
- Uses technology for the completion of specified job duties.\n
- Provides back up coverage for other nursing personnel within the Agency.\n
- Reviews and monitors medications in order to ensure that an adequate supply of all prescribed drugs is available.\n
- Maintains all records in accordance with established protocols.\n
- Receives and reviews input from staff regarding the health needs of the program participants.\n
- Provides emergency, on-site treatment and triage for Vocational Services consumers.\n
- Assumes other duties, responsibilities and special projects as needed.\n\nSecondary Function(s)\n
- Schedules all routine appointment.\n\nREQUIREMENTSEducation : Graduation from an approved nurse training. Licensed as a Registered Nurse in the State of DelawareExperience : Two years of experience required, preferable with ambulatory services for persons with cognitive disabilities.Ability to lift over 50lbs.
