Job description

We are seeking dedicated and compassionate individuals to join our team. The successful candidates will be responsible for coordinating and providing comprehensive care management services to our clients. This role requires strong organizational skills, empathy, and a commitment to improving the well-being of those we serve.

Responsibilities

  • Develop and implement individualized care plans for clients.
  • Coordinate with healthcare providers, social services, and family members to ensure comprehensive care.
  • Monitor client progress and adjust care plans as necessary.
  • Advocate for clients to ensure they receive appropriate services and resources.
  • Maintain detailed and accurate records of client interactions and progress.
  • Provide crisis intervention as needed and connect clients with emergency resources.

Qualifications

  • Bachelor's degree in social work, psychology, or a related field.
  • Previous experience in case management or a similar role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage multiple cases effectively.
  • Knowledge of community resources and social services.
  • Proficiency in Microsoft Office and case management software.
